MSF does not have access to all student records. For card replacement, you need to contact the site where you took the course. In order to obtain your training site information, you might try contacting your State Coordinator or Military Coordinator (Listed Below).

Cards issued more than six years ago can not be replaced - attending another training course is recommended. If you completed training through a civilian training provider, please contact that site for a replacement card. The Air Force Safety Center does not maintain copies of course completion records.
